Turning Out For a Sport? Here's What You Need To Do!

How do you get a "PINK CLEARANCE SLIP"?

  • Purchase and ASB Card $45.
  • If turning out for a non-cut sport, pay $75 for high school and $35 for middle school participation fee.
  • If turning out for a cut-sport (cut sports include Volleyball, Soccer, Basketball, Fastpitch, Baseball, Girls and Boys Golf), you won't pay participation fee until you make the team.
  • Pay all fines at ASB Office.
  • Complete an Athletic Participation Packet (see below) once per school year, and a Yellow Emergency Card.
  • Have current physical on file (must have had physical within last 13 months).
  • Bring Receipt from ASB Office, Athletic Packet, Yellow Card Emergency Card (unless current year on file.
  • If you are a home school or Running Start student, you need to complete a WIAA Eligibility Contract. under "Student Eligibility and Related Issues."
Download and sign all forms under 1-4 below.

1. Athletic Participation (Form 110) and Dr. Physical (PDF)
  • Demographic Information (proof of residency in South Kitsap School District)
  • Proof of Insurance (company, policy # and parent signature)
  • Parent permission (parent signature)
  • Activity pledge (student signature)
  • Risk of Injury (student signature)
  • Risk of Injury (parent signature)
  • Medical history completed (parent/student)
  • Current Sports Physical (within the past 13 months)
2. YELLOW SKSD Emergency Medical Card (PDF) (needs parent signature)

3. Student-Athlete Checklist (PDF) (needs parent and student signatures)

4.Read information pamphlets regarding:
Print and Sign acknowledgment
5. ASB Card
6. No Fines or Fees (receipt from ASB Office)

7. Participation Fee:
- Senior High - $75.00, if this is a second sport or activity for the year it will cost $40. Third sport or activity for the year is free.
- Middle School - $35.00

Please help the high school athletic medicine staff with getting your athlete's correct medical information. The athletic medicine staff has set up a website where you can enter your athlete's medical information along with other valuable information including injuries, allergies and medications.
